Puppy with rash, hair loss, red eyes .

My 5 month old puppy has a rash around her mouth and on her eye lids.

Her hair is gone in these areas and the rash is red and looks sore. Her eyes have matter and are red.

A rash and hair loss could indicate allergies, infection, mange or other possibilities.

As it is on her muzzle and face, my first guess would be mange. This is caused by tiny skin mites and can be diagnosed by your vet by taking a skin scraping and is treated with special baths or dips.

However, as her eyes are red and they seem to have some discharge, she probably has conjunctivitis (an infection of the eye) in addition to whatever is causing the rash. This will probably need antibiotic treatment.

I'd recommend taking your pup to the vet as soon as you can so that she can get correctly diagnosed and treatment started.
